-
Print the blank form.
-
Complete offline. (Write out the info by hand so that later you can enter the content on-line, OR type your responses into a text document so that you can cut and paste sections to the appropriate spaces on the on-line form.)
-
Return online to enter your prepared materials.
NOTE:
For any supporting documents that you would like to send as attachments, please label your attachments as [YourName][What-it-is].doc (e.g., "MilgramConsent.doc", "BeckSurvey.doc") and email them to:For Frequently Asked Questions, see FAQ.
If you do not know whether your project should be submitted as Exempt, Restricted, or Full Review, then go back to the Inside Bard IRB page and complete the diagnostic questionnaire.
Renewal requests and minor revisions to approved protocol may be reviewed by the board shortly after they are received. All other proposals will be reviewed within two weeks of the most recent submission deadline. Proposals submitted after the deadline will be reviewed at the NEXT scheduled meeting of the board (usually the following month).
