Accident and Health Insurance Plan

YOUR FAMILY/PRIVATE INSURANCE IS PRIMARY FOR ALL OFF-CAMPUS MEDICAL SERVICES, MEDICATIONS AND SUPPLIES. It is your responsibility to familiarize yourself with your family/private insurance, i.e.: if it is a PPO or HMO, you may need to find a participating provider and verify benefits. To find a provider in your network you must call member services. This telephone number can usually be found on the back of your insurance card. This should be done before you schedule an appointment otherwise you risk benefits being paid at a reduced rate or not at all. In the event of an emergency never delay treatment, go immediately to the nearest hospital. In this case your insurance should be notified as soon as possible. If you have any questions, please call the health and counseling service administrative assistant, at ext 7077.

If you have no family/private insurance the following applies to the Bard College Blanket Accident and Health Insurance Plan.

PLEASE NOTE: The Bard College Blanket Accident and Health Insurance Plan (Markel/POMCO) has a number of exclusions. Please refer to the Bard College Accident and Sickness Plan (PDF) for more information.

Payment of fees:

Payment may be required at the time of service but some providers may file a claim on your behalf. This will need to be discussed with the provider prior to the first appointment so you can be financially prepared. In either case, if you are going to use the Bard College Accident and Sickness Plan insurance a claim form will need to be filled out and submitted to the insurance company in a timely manner.

Claims Procedure:

To file a claim under the Bard College Blanket Accident and Health Insurance Plan, you should:

Print out a copy of the claim form (PDF) or pick up a copy from the student health service or call ext. 7433 to have one sent through the mail.
